FOR THE APPLICATION FORMS.
To invest in UTOS, please note the following important information.
- Full payment of units is required with each application (25 units minimum).
- Payments can be cheque, cash, or money order for which a receipt will be given.
- Applications can be lodged and paid at the Gambling Control Authority Office in Salelologa, Savaii.
- Gambling Control Authority Offices do not provide financial updates on investments but receive applications on our behalf and receipts purchases of units.
- If this Application Form is signed under the Power of Attorney. A certified copy of the Power of Attorney should be submitted with this application for our records.
- Applications must be signed personally. Applications by unitholders under the age of 18 should be signed by the authorized applicants’ trustee/parents/guardians.
